Simple data entry tool for Microsoft Word template.

Hi guys, I want to make a very simple program that enters information into a Microsoft Word template. It is very simple and I have done all of the design layout already, it just need coding. This should be easy to develop and does not need to look special at the back end. I have attached 3 documents. The Entry Example shows you how the information will be entered. The Marking Example (Fields) shows you where the information goes The Marking Example shows you how the final printable product should look. The idea is that I can use the program that you develop to tick a box next to an option, and then text relevant to this option will appear in the correct place on the Microsoft Word Template. Most of the fields are empty and you can fill them out and use the check box next to them to select them. Fields are restricted to a character count stipulated by the layout that I have created. The only other requirements Is that you can save the layout and text content each time you use the software, or load previous versions. Look forward to hearing from you guys. This should be quick and easy to create. I am not looking to pay a lot for this guys, certainly not in the tipple digits. I will award to the lowest bidder and will leave excellent feedback. This software is just for me to use and I will not be sharing or selling it.
