Daily Attendance Checkin for Database (optimized for iPad)

The project is to create an attendance checkin page that is optimized for iPad and iPhone (or other smartphone). The page needs to pull all clients names and ids from a database for a certain boot camp date (based on a GET variable in the URL). It should display all names with a checkbox next to each name. The end user (instructor) should be able to check each box (based on attendance for that day), and click submit or save. Upon submission, the page should insert a single record into the database for each client. Each record will contain only three fields. The attendance_id (auto incremented), the client_id, and a date (auto date/time stamp from mysql). The list of names should look good on iPad and on a smaller device (perhaps 3 columns of names on iPad, and 1 column on iPhone).
