wordpress website - database and qr code

MySQL database to store the following data: - id (number) - QR (image) - pet image 1 - pet image 2 - pet image 3 - pet name (string) - pet breed (string) - pet weight (string) - pet gender (male or female) - pet description (string) - pet behavior (string) - pet health (string) - pet notes (string, optional) - owner image( optional) - owner name (string) - owner address (string) - owner's phone number (string) - owner image 2( optional) - owner name 2 (string, optional) - owner address 2 (string, optional) - owner's phone number 2 (string, optional) - owner image 3( optional) - owner name 3 (string, optional) - owner address 3 (string, optional) - owner's phone number 3(string, optional) - owner image 4( optional) - owner name 4 (string, optional) - owner address 4 (string, optional) - owner's phone number 4 (string, optional) QR code generator, WordPress page with a button "generate QR code" ; new QR code will be generated with a new database entry, and unique ID, with an option to download a QR code or send it by mail. (The new database entry will store the QR code image and ID.) When the customer scans this QR code it will find a database entry with that QR code and ID, and will check if the entry pet name is empty: if it is empty: open a page to fill in all the entries listed above, and button "save" to save data (text boxes, image upload box, etc) if it is not empty: it will open a page and show all the entries listed above in a nice design (images as slide show...)
