Simple Questionnaire / Form Setup & Design

Hello, I am looking for a talented freelancer to help build a series of forms. The form will have the design/feel from the link seen here: [login to view URL] Some key points: -We will provide all the questions, options and formats for the form. -We will provide the images and code from the current layout. -We will want a few different forms with a variety of questions to be built. -Forms should be editable with a basic understanding of HTML. -Some questions may be formatted into columns. -Form should send email to a predefined address and send a thank you email to whomever fills it out. Again – I have basic understanding of HTML and I wanted to be able to edit these forms and build new ones once a framework is in place. The successful freelancer will be whomever responds with an answer to how I can have a system to build and scale these forms using basic HTML formatting.
