Outlook Reservation Form

I am looking to design a simple reservation (appointment)? form for outlook.? I need the form to have a pop up date calendar and drop down lists that I can change the content over time.? The form is very basic in design with the exception of the pop up calendar.? I need? the form? to be compatible with outlook 2003 ond 2007 and I also need to be able to install the form on our exchange server.? I would also like to store the name in the database and be able to have a name recall for repeat customers so that we can have easy access to their history.?
