I have a complete ready form created that needs some fine tuning. Some fields needs to be required Some fields needs the class=money In the review section headings of each item should be visible even if its filled or/and it doesn't have to be editable as its a review page, so it can just have the heading and field, like it'll look like in the email... The email that admin will receive needs complete editing to look presentable and readable (maybe like the Review page). Add a reset button just in case to start over.
