PHP MySql enrolment system

We run a car rental company and need an enrolment system to add extra drivers to our existing company customers. Situation: We have a number of large companies that are using our services (Car rentals). Each of their employees / drivers are currently manually entered into our b.o systems and we confirm each new employee manually with the company representative (validation of drivers needed). We are looking for someone to develop a system where our customers employees can sign up to use our services on our website and have their signup confirmed by the company contact person. 1. Create a website where we can set up new companies with basic data (including contact person) and with several invoicing units and each invoicing unit with several cost centres (three levels). Including MySql database and tables (Database available) 2. Create a page where the signing in user enter a company code and when pressing enter are taking to a page with company information. 3. On the page with company information the user should be able to choose (when applicable) invoicing unit and cost centre and enter some required personal information. When submitting, a "confirm signup" email should be sent to the company contact person. Email should have a link to a page to approve the signup. User should be informed that the email has been sent, pending approved and displayed the submitted data. 4. When the company representative clicks on the email link he/she should be taken to the confirmation page where they can approve the users displayed request. 5. When the user is approved an email needs to be sent to the new user and also to us with the information to the customer of some basic signing in information and an attached pdf file. A semicolon separated text file should be able to be created for b.o system import. Sample php pages has been created for information purposes (I am not a great coder so just for guidance), we have a designated MySql database for this. a dreamweaver template and css file will be handed out to the coder. So, a fairly small project, we are looking for a very quick job done (this week delivery appreciated) Final language is swedish so easy editing of text fields / emails would help.
