HTML / PHP contact form with 2 file uploads

I need a contact form created on my website. It needs to have 2 upload fields. Once the form is submitted it will email me the message and the 2 uplaods will be sent as an attachment. it needs a few text input fields and 1 drop down. You can reuse code if you've done this before.
