I am looking for a php script which will calculate body mass index, caloric need and capture items that can be stored in a mysql database The user will enter inputs and the result will be displayed in a self contained stylised box on a html or php page. Upon award full detailed layout will be provide and which elements used to store in db. The results should be mailable to the user if they enter an email address also as well as being displayed. The request to mail will have a checkbox and area to enter email ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows? (depending on the nature? of the deliverables): a)?
