Hello, I need this form to schedule an appointment on the website. Site is joomla based, so it would be good to have this as a joomla module. This form should send emails to both parties and create a database record. Admin should be able to see appointment requests and confirm the schedule or modify the proposed time. Once changed or confirmed should sent an email to customer with corresponding data. Customization will be done based on css style sheets so, plain html5 will be fine.
