Design of a maquiladora app mockup

We have developed an app in php and need a good design for it. In the attached file you will find the actual screens of the program so you can have an idea of what we need and send us a sample of your work. The design must be in html so we can integrate it to our app directly. Preferred colour is blue. creation of a logo for this maquiladora software would be nice.
