Fix PHP form - a couple minor problems

Hi- My coder created a form that was working, but he did a minor change and something happened and the email function is not working. It is late where he is now and I need this form working in the next couple hours. A couple things need to be done: 1. Fix it so that the email function works. I can send you the old file when it worked, the problem with the old file is that we didn't like the formatting. The new file has better formatting but it isn't working. 2. Fix what happens after you submit the form. Right now, it shows everything on the form that you sent, and then way at the bottom is a "thank you for applying" screen. I don't want the user to view the completed form, I just want it to go to a "Thank you" page. I would like the text in the "thank you" area to be smaller than it is now.
