Custom Outlook Form - Appt

For Outlook 2002 with Win XP: I need an Outlook form that is based on an appointment form and that will let me enter enter information into a public folder (WorkgroupShare)and obtain reminders from the appointment: There are several custom fields we want to be incorporated into the form, in order to track upcoming bids. It should have all the regular task options that come with the standard Outlook form (such as setting categories, priorities etc.) ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Installation package that will install the software (in ready-to-run condition) on the platform(s) specified in this bid request. 3) Complete ownership and distribution copyrights to all work purchased. ## Platform Windows XP
