Simple business listing script

Simple project - A php script that allows the following: A user fills out an online form that includes an image upload field. Data is stored in mysql, image is resized and cropped (GD2 / ImageMagik) and also stored in the db. (or folder) Data is displayed on relevent webpage according to catagory etc.. (existing php pages) Form to include a basic captcha. No registration is needed by the user, so each application needs to be approved by a basic admin page before being allowed to display on a public page. Admin page also to include sort / edit / delete entries. Each application also to send email notification to admin & user. So --- Data in from form: Title, Description, Location, (select from dropdown) Catagory, (select from dropdown) url, email, image, Resize-Crop image / store data. Retreive data for specfic locations / catagories etc...
