PDF split and labeling

I need a function that should do the following actions. In a PDF multi-page document to search for a predetermined position in each page and select a fixed space up to the right into the border , that always include a text of the kind no&date i.e. 134 and 15/10/2011 where 134 is an auto number and the date in the form dd/mm/year that is ID field (explained below). What I want from the function is to search for an equal ID in a ACCESS query that has fields Patient and ID, split each page as a separate PDF and put the correspond Patient as a label on this separate PDF with the addition of a_ and a number (common for all PDFs) that should specified by the user. I can supply with the PDF where the coder should see the exact position of the ID and also I could change, by hand the label on a PDF, in order for more accurate understanding of the function I have to point out that I don’t pay for time only for job Also I don’t release any milestone before see progress.
