189960 easy html job

i need some help solving 2 issues i have... i just made a new design for my site at [login to view URL] however when i copied the contents of the order page to the new design now it gives an error when trying to order.... you can TEST IT AT [login to view URL] Also when i copy the contents of the meta tag generator on [login to view URL] when i try to use the program it gives an error... I can give you original [login to view URL] and [login to view URL] files where these things worked before i changed the design which should be very helpful since i think i just missed a peice of the code when coping the contents
