Design a form in Word from supplied document

I have a questionnaire for clients in Word 2007 which I need someone to make look very professional and turn into a form using the various form elements available in Word. You should add text fields, checkboxes, dropdowns and any others you feel would be useful. We will need to email this form to clients to fill in so it needs to be foolproof so the fields need to be editable etc and they shouldn't be able to delete the questions etc. I would like the person to look at the overall design including the logo at top and footer for improvements and make sure the form is simple to use. I stress this needs to look very professional and want a designer who will make this look great.
