HTML expert + form + basic admin to export sign ups

Hello, thanks for looking at this bid request. We are promoting a competition. It will be promoted via email and social media and encourage people to click through and enter the competition. I need someone to help with the creation of the following; 1. HTML page, which is very basic and an example provided. Please note, there is no PSD. This is a simple webpage template. 2. We then need to use that template for 6 x forms.. essentially 6 pages, with a form (fields will be provided) 3. Entrant's details needs to be collected. 4. We need a secure area to login and export the contact details. 5. When a user submits the form, they will go to a thank you page. There will be a thank you message and links to the other 5 competitions. Ideally you will have good feedback, anyone with zero feedback will not be considered.
