I need TWO multi-page checkouts for an online certification program . ***VERSION ONE*** On Page 1 they will input their name, address etc On Page 2 they will choose their service. Services are offered with a 1 time fee or monthly recurring On Page 2 they can ALSO choose to do a custom service amount so if none of the standard service selections fits their need , they can create their own service plan. On Page 3 they receive confirmation of their order before hitting the submit button. Confirmation includes their name, address, and service plan chosen. On page 4 they can choose to pay via credit card or echeck through [login to view URL] This can be a one time payment OR monthly recurring payment. Form results are sent to my email address AND customer receives a confirmation email. ________________________________________________________ ***VERSION TWO*** On Page 1 they will input their name, address etc On Page 2 they will choose their service. On Page 3 they receive confirmation of their order . There is no payment associated with VERSION TWO. It is an order form only. Form results are sent to my email address AND customer receives a confirmation email. ***For both versions, I would like all form responses downloaded into a database automatically. *** For both versions, Once the customer hit the submit button, they will need to receive a customized pdf certificate with their name and program choice on it. This certification is something like an online school or certification program would send.
