Wordpress Website Design

'm looking for someone to design me a 2 column Wordpress website for my internet marketing consulting business, I will require a website with 8 - 10 pages, I will write all website copy. I will require 2 sign up forms, one on the top right hand side with strong call to action for collecting names and email addresses for sending a free report, I will also need a graphic for free report. I will also require a free consultation sign up form at the bottom of the home page, with multiple information fields. I would also like my company logo used in the header of home page and on all other pages with in the site. I will require a twitter, facebook and youtube logo, one of each, and have them linked to my social media pages. Here is a website that has a similar feel, to be used only as a reference [login to view URL] I would also like to integrate the logos/graphics of large companies that I've worked with into the footer/bottom of home page, similar to the website above. I will show you the logos of companies that I've worked for you to copy. I will be adding a video to my site at a later date, (similar to site above) so this will need to be taken into consideration when designing the site. In terms of design, I would like a clean professional design that fits in well with my company logo. I would like to have this up and running in 2 weeks time.
