Need hopefully simple Macro for Word to format a document

We use a program that manipulates MailMerge fields to generate report documents from Word template forms. Once the document is generated, there remains some formatting to be done. The program we use doesn't allow macro-enabled word files as templates so it needs to be an "installed" macro run after the fact.
Would like a one-click button in its own section of titlebar (I'm sure that's not the correct terminology) that does the following:
- Popup window asking users to select their office from a list of 18... can be radio buttons or dropdown, don't care
- Set a text field in the document header with the office address and phone number based on the selection on the popup
- Format images and tables- I already have a working macro for this so feel free to reinvent or I can provide what's working
- Needs to include means to install and uninstall
In a perfect world, I'd like to somehow also insert page breaks at a specific repeating portion of a table but this might not be something Word is smart enough to identify.
I'm a mechancial engineer not a software guy so please bear with me, I'm sure I'm not asking all the right questions or giving all the needed info at first pass here.
