Wordpress Calculator Plugin Needed

I am in need of a skilled developer to create a wordpress calculator plugin. This plugin will be for a text message marketing company. The will recommend what texting plan the customer should choose, based on how they answer three questions. Once the customer answers the three questions, they will see what plan is recommended. The plan should also be clickable, so the user can order. This must be a wordpress plugin. I should be able to add a shortcode to get the form to show.
