2 Responsive Webpages need to be created for a project

I’m working on a project which will be used to track internship/work logs for the university and need a couple of completely responsive webpage for the same. HEre’s the requirement: We have a login page created where the user logs in and the user information will be stored in the university database to which my login page is already linked to I need to create a webpage which will later be linked to a database which will later populate data The first page should consist of the following, which will later be added to an existing navigation bar as a tab - NAME of the Tab would be Internship information CPT ID Student ID Client ID: Instructor ID: Term ID: Supervisor: Job Description Note Active Total Hours Each field requires a text field so the user can put in the information and once a user is done giving the inputs, the data needs to be saved in a database. If the student is a returning student, then these fields should get auto populated with an option to update/edit the information The second page should consist of the following information, which will later be added to an existing navigation bar as a tab - NAME of the Tab would be Week Log information - Here in this page, a user inputs daily logs where user reports his/her daily activities CPT ID: Week No: Monday: Tuesday: Wednesday: Thursday: Friday: Other: Final Each field requires a text field so the user can put in the information and once a user is done giving the inputs, the data will be saved in a database. here’s a SAMPLE page with a mix of both my requirements, I will basically need to split this page up into 2 pages, This url is a SAMPLE only and the work cannot resemble this example page Here I'm also attaching some picture for clarity purpose.
