PHP script needs changes

I have a php and css code that I need adjusted as follows: 1) modify the upload in CSV section so that each time I upload a CSV file it displays all column headers and ask me which ones contain the information needed (i.e. first name, last name and domain) in order to create valid permutations 2) in both the csv upload and manual entry, we will change the email validation from the current method to using an api service. Results will be displayed using a green, yellow or red rectangle icon instead of current checkmark or cross icons. If results are unknown (yellow) then we stop using the api service as all emails for the domain will be unknown. 3) results from the above will be stored in a mysql db depending on the results from the api. If results are valid then we will save it in the DB together with first name, last name, domain AND we will create an email format for these valid emails. Email format will be like [F1]@[login to view URL] or [F1].[L1]@[login to view URL] and so on. If email is invalid then there's nothing to save. If domains is unknown then we will store just the domain name and mark it as either gray or catchall. Looking for someone that can code clean and completely commented code. Also must be able to start and complete this project within the next 4-6 hours.
