need gravity forms [login to view URL] and [login to view URL] setup to mimic what the old form did regarding recording of data in database plus a few extra configuration details on the gravity form, i.e. dynamically set application fee on membership application form to auto fill depending on whether applicant is from USA or international. Old form code files attached. Need setup to record correct database data for broker-course-registration-form to place data in the tables and use of correct fields id and other field values on database for online-course so registered user is set-up to begin course. After setup of membership-application to record correct database data correctly into corresponding user and cimy-User-Extra-Fields database tables with correct field id's and other field values.
