I need a custom woocommerce order form built. The design should be exactly the same as the picture attached. See this site [login to view URL] The form contains the following information: 1. Where should we bring this item? 2. When should we deliver this item? 3. Your full name 4. Phone number 5. Email address 6. An optional tick box For the delivery location. There is a google maps background. Which changes depending on the location you type in. It would be cool to have that. It would also be cool, to limit which post codes can be selected. Theme Details: Jupiter Theme Plugin Details: Woo Detail Product Page Builder I'm using this woo detail product page plugin, to completely change each woocommerce page. So that it does not use the standard woocommerce layout. But instead uses something similar to the theme. Please make sure you understand how to customise theme's which use this plugin.
