I would like a very simple contact form created, this contact is to include multiple file upload fields (not HTML5 multi file uploader).
I have a rough template attached which is a guide to the page I want created.
Upon submission the form will send to an Email address listing the contact information of the person submitting, the files will be uploaded to my server with a unique file name, the Email I receive will have a <img src= to their location and the images will be displayed in the Email so the Email I recieve will be in a HTML format.
