5 Page Website w/ wordpress and forms - Highest Quality Finish and Design Only

Project description: This is a project for a fairly simple approx. 5 page website with wordpress installation and two basic forms. Context: A complete website for information on laser eye surgeries. Detail: I have LasikExperience . com and have set-up a VBulletin Forum on the subdomain [login to view URL] . com. I like the background, foreground and tab look of nelsonmandela . org. (except, I would like it without the right hand column). Different color palates to produce a similar looking site are ok. 8 tabs are required. Tab Home) should contain wordpress. Wordpress postings should each have ability to Comment, Email, Print, Share: Digg/[login to view URL] Homepage should have a sitesearch box, RSS add and newsletter sign-up under the tabs or off to the upper right hand. Hence, relevant graphics/pic will be required for the home page which I will help select or provide. Tab 2) The forum should go here and be tweaked / customized to blend with the template. Tab 3) Will contain a quiz such as the one on seewithlasik . com/docs/[login to view URL] (Different but similar content). The quiz should have radio buttons for yes no answers and for those that have multiple choice answers, the interface should be in the form of the drag bars such as the 'Find Professionals' feature on Elance. A big box should displaye a score based upon the input like calculated on the seewithlasik site. I should be able to add to the selections here. I will need to tool/necessary process detailed to do so. TAB 5-8) Remaining tabs will contain static content (most likely bullet point lists) I will add later. They should be designed in a way that they can be long lists and be printable cleanly. Optional: Pls quote seperately Logo: I can send you a simple stetch or you can prode some mock ups. Support: Also pls quote me seperately a support contract for 3months for minor tweaks/maintainence. Key Success Factors and prior experience required: -Website must be simple, clean, uncluttered. -Design must incorporate best practices for scalability and duplication. -Overall look must be polished. -Attention to detail is a must. -Pls no violation of copyrights. -The site should require little or no maintanence. -I should have all admin tools AND PROCEDURES to do minor maintenance myself (I have basic knowledge of HTML).
