Module to fill order forms on web (intranet) refDPSSS

(ref:DPSSS) We need a PHP GUI of approx 3 pages to insert records to a master detail table structure. This will run on the intranet of a retailer where branches may send goods to each other. The application is about storing these shipments in a certain format. The references of the shipment (date, location, id etc) in a master, while the details (item, quantities etc.)in a detail table. All the other constraints and the db (MySQL) structure are in the attached document. As this is part of a bigger project, PHP coding, use of CSS is a must. The test document will be provided to help the coder to submit his work. The db access, and an ftp to test the code will be provided to the coder. The final code has to work on our ftp. The program will work in Turkish therefore windows-1254 or 8859-9 sets are to be supported ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased.
