Updates to existing Shopify theme

Hello there, One of our clients has a website built on Shopify and we need to do the following updates: 1- Set an existing page as the default page. We need to hide the menus. We need to create a small contact form as well. 2- Create a page with a new form with the following fields: Hello! Thanks for your interest in carrying our products! Please answer our wholesale application and we'll contact you shortly. Company Name: Contact Name: Phone Number: Email Address: Shop Address: Shipping Address: Type of Store (Online/ Brick and Mortar/ Other) Tax ID: How did you hear about Luvaboo? Comments Submit 3- Create a category page with a grid-like presentation. Two rows of three fields per row where we can customize the content. 4- Embed a PDF file so that the content is fully visible without needed to download the file. 5- The URL is found in the txt file attached.
