Flash slider to post to SQL DB

We need to create a way using flash for users to input data. We need to re-create our user input using sliders which allow users to score 12 critera. Users assign a total of 60 points over the 12 criteria. Each of the 12 criteria can be scored from 0 to 10. Again the maximun is 60 points. The information needs to be written to the database in real time. The backend is a mysql DB. Please contact me with any further questions Please see attached image.
