I need someone professional who can create for me a Fully Automated Questionnaire form which Tallies/adds up Scores provides results and Sends clients a completed Analysis. As well I would like a way to export the clients contact information to place in my Client Database. I have created this Questionnaire which you can view on my website at [login to view URL] I used [login to view URL] to complete this. As well as it works the issue is I still receive the completed for which I must add up the scores and then I have to put an analysis together which is already prewritten but depending on the score it will send an analysis with different prewritten information. I have upload an Analysis for you to see what it looks like and I have also upload all the information that would be included in the analysis depending on the scores. I would have to explain in more detail how I put the Analysis together but it's very simple. I have too much work and getting too many leads to completed all these Analysis' that I am behind so if I can have it automated where the client will get the Analysis immediately in their email that would be perfect!
