Please create a Jotform template that will collect the information in the pdf shown in the link below. This will allow this form to be filled on using my Jotform account online. No special logic for the first version of the Jotform created but additional logic dependent on how the questions are answered will be added in subsequent work. Only the first page of the PDF in the link below needs to be converted to a Jotform template. More work will follow for creating online forms using Jotform for a vaccination clinic if this first project is successful. [login to view URL]
