Create a Booking Form on WordPress with Payment Integration

We are looking to build a sign up form on our website. The sign up form must allow users to sign up for our service. The user will have the option of: - Paying for a Trial - Paying for a block of time When the user selects to register for a block of time, they will be able to select a fortnightly payment option or they can pay in full. We currently have a WordPress website running WooCommerce. We are not after a calendar function but we have been told WooCommerce can be extended to support this however you are free to user other plugins such as GravityForms and or Woocommerce Subscriptions etc. Each 'service registration' will correspond to a particular location and the amount that is billed for a 'block of time' will change given we must be able to adjust how long the block of time is. For example, at the start of the program, the block will be for 10 weeks however half way through the program, the user can still sign up for the remaining block of 5 weeks. Attached is a wireframe of the form with the minimum data. We would also like to give the option of conditional logic when they select 'Yes' to adding another product during registration. We have also heard of EventEspresso able to meet the needs of this so it may be a matter of using your skills to configure this and meet our needs. Payment was be able to made and integrated with PayPal or eWay.
