Custom drupal module

I need a custom drupal module coded up. It will allow me to get customer feedback without them being members. What do I mean? It will work like the ebay feedback system. I will give a special link to a customer who uses our service, they will rate us in a few categories and write a short feedback about the business. This is then displayed in a block on the main page similar to ebays system. A summary will also be displayed of how we are going. the module needs to be able to give out tickets so the form can only be filled out once to the relevant participant. Thanks.
