auto reminders using ivr and email

i would like a application built which can read data from database and would like to send out reminders for their clients. they will send information such as office id appt id customer id customer name customer pin customer appt date customer appt time customer appt with name customer special instructions customer appt reminder with email y/n customer email customer appt reminder with phone1 y/n customer phone1 customer appt reminder with phone2 y/n customer phone2 other we need to keep track of patients responses on these calls. if patients wants the appt to be private, we will ask for a pincode to be entered first before details of appt are given. the customer can then enter 1. to acknowledge the appt. 2. to reschedule the appt. 3. to cancel the appt. 4. have the office call them. 5. acknowledge the reminder but remind again if this reminder is less then 24 hours from appt. If possible i would like to have option to SMS on patient's mobile from the same. completing the project in vb maybe the best option for our inhouse programmers. you must have access to multi-modem dialog card from intel.
