Easy/Nice Interface for entering in Applicants into a database application. Similar to a contact manager, this project will require the need to store all information being entered include the date in which entered. The initial fields will be: First Name, Last Name, Address, City, State, Zip Code, Home Phone, Work Phone, Email address, Members Age, Members Gender, Spouse's First Name, Spouses Age, Spouses Gender, Dependent 1 First Name, Dependent 1 Last Name, Dependent 1's Age, Will need up to 6 Dependents, Requested Effective Date. This information will be used in the creation of many reports. Only the basic report will be required for the first stage of this project. This Report will only show the information entered in a professionally formated letter. Chrystal Reports would be perfect (we have a license for Chrystal Reports V10pro). Data Storage: All data should be stored in an Access Database Backend and should not be able to be opened without being in the developement environment. Provide a lookup form for past stored Applicants. Applicants will need to be stored in a seperate Access Database. Another Access Database for storing owner information. Information on the plans will be stored in a Third Access DB. Serial/Password protection with timed shutoff because this will be a monthly renewing product. Serial will need to be based off hardward in the installing computer. A partial combination of the NIC Serial number and Serial of the primary HD in the system run through an algorythm would create a harder serial based code to crack. Will need a serial generator, prefer to have that in PHP so can be web intergrated. This is not a high requirement as long as an algorythm is given to me, I can create the PHP scripting. I would love to see the timer use the an Atomic clock and would require the user to log into the Internet every 7 to 10 times to verify the softwares eligible for use. However I'm open for suggestions.
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):
a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.
b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
