Application Form & PayPal Cart Integration

Hello, I am having trouble piecing together an application with an order from that will send the desired amount to PayPal. As you can see with this order form: ][login to view URL] We have created Javascript calculator that will show the price in box #1 but also in box #13. We can not figure out how to 1) send the form contents to the client and at the same time generate a PayPal cart *with* the price and service level retained. Here is a graphic to demonstate....[login to view URL] (the price is not accurate, it's just a visual) IF the customer does not select PayPal, we static pages to result (either check payment or phone order payment pages). Can you give me a quote on making this form work 1) Emailing the contents to a designated email address. 2) Checking out using PayPal if the customer selects it in box #13 3) Making the submission of the form generate static pages (I will make them) for the check and phone options?
