BarCode Reader PHP Database

I need a script in PHP using Mysql where we import our .csv file and each import will have a unique ID which will be pull from that .csv file and that will generate a barcode so when we scan that barcode using that script it will show the information based on that .csv file, its a fairly easy job. Things I need for that is User/Password Access Barcode which you generate must be readable by a barcode scanner. Search able form field based on that unique ID which we will import in .csv file so what ever headers we will import those will be the field for that script, there will be about 20 fields if not more. Let me know when and for how much.
