aspx form to build and send email

I need some one to add vb.net code to a form on an existing aspx web page design that I have. The web site I have is on a Windows 2003 platform. The page is to generate a simple html email to send to me, populating the email with data entered by the user. I also need the form validated with standard sort of validation such as disallowing blank fields etc. before a submit button gets executed. Also need (for example) disabled radio option buttons to become enabled if the user clicks on a particular checkbox, and then the user must make a selection before the submit button generates the email to me. I will give the successful bidder the design of the aspx page in which to add the code, plus exact detail of validations, fields being enabled etc. ## Deliverables 1) Complete and fully-functional working web page in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment (ie. "live" on destination website). 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased.
