Build an online dashboard that can be accessed via desktop and mobile/app

We are creating an online membership program for mothers and their babies which will last four weeks. We are using a WordPress site and the Member Mouse plugin. As part of this program, we want people to be able to input details about themselves daily or weekly and their progress and record it on a dashboard. We also want this dashboard to show them the averages of everyone else in the program so participants could see how they compared to others. We would like this dashboard to be accessible from a desktop computer but it will more often be used on a mobile device so it needs to be configured as an app or with an app-type interface. There would be the following fields which would be entered once only and some of this information would need to be captured through the person's Member Mouse submission: 1. Name of person 2. Name of baby 3. Age of baby in months 4. Weight of baby There would be the following fields which would be entered on a daily basis: 1. [number field] Total daytime hours your baby slept in last 24 hours 2. [number field] Total night time hours your baby slept in last 24 hours 3. [number field] Total day or night hours baby slept in last 24 hours 4. [number field] Total number of bottle feeds 5. [number field] Total number of breastmilk feeds 6. [number field] Total number of solids feeds 7. [pull down choice field] How did you feel today: terrible, a bit flat, average, good, fantastic 8. [number field] How many hours' sleep did you get in the last 24 hours 9. [pull down choice field] How was your baby in the last 24 hours: sick, well There would also need to be on the dashboard items that we would control: 1. Today's date and what week they were in the program eg Week 2 2. Their membership level and link to account details 3. A tip or message of the day The dashboard would need to be branded with our logo etc and have the same look and feel as the rest of the website which is under development.
