Create html webform based on infopath form

I have an infopath form that is attached here. I need someone to look at this form and create an exact copy of it in html. you will notice that some sections require the ability to repeat, this essectially means when you hit it, the fields in the container will duplicate. If you have infopath running on your pc you can see this functionality. If you dont have info path this project is most likely not for you. On submit this should send to some xml file on the server with the name and date as the file name. I dont need any validations at this point but i do need it to look and act like this infopath doc. be sure to catch some of the css styles that you see when you rollover things. this is required to.
