Create html test with social networking

I have two flash online tests (simple). I want them converted to HTML (php, cgi, I don't really care) 1.) The user will enter their name and email address to enter 2.) They will fill in all fields, check boxes 3.) it will calculate a score (results) 4.) The results with user's name and email address will be emailed to me (or saved in a database) 5.) the user can share the results to their facebook or twitter profile.
