Custom Form Templates for Microsoft Outlook 200

We are requiring Custom Form Templates to be designed. This will be used instead of the default customer form that is installed on Microsoft Outlook. ## Deliverables We are requiring Custom Form Templates to be designed. This will be used instead of the default customer form that is installed on Microsoft Outlook. We sell a wide range of products into different industries. We are wanting one form that can be used for all inquires, wether it is an inquiry or an order, from new or previous customers. So when an inquiry is received via phone the appropriate box is ticked that the customer is inquiring about and the information that we need to complete about them and the product their inquiring about becomes visible on the page. Basically a phone script type of questions we need to ask and to be able to mark off the answers that we are given. Put in dimensions and have a product estimate calculated from the information entered. So a small calculator, with basic calculation needs to also be available for certain products so we can work out from the information we receive and enter how much product they will be needing. If information is needed to be sent out, via email, fax or post then these can be done directly from the form and sent immediately. If an action is needed by another staff members then this can also be added and then assigned to a new employees and passed around until the tasks are completed. When anyone else opens up this contact and adds new information need to have date stamped and initals box so we have a timeline of activities and contact with the customer. Also need to be able to take orders, product information, shipping information, special instructions and payment details if they decide after receiving information that they want to place an order. Some of this type of information can be hidden on the page until the 'Order' box is ticked and the order details become visible. We already have several fillable pdf forms we currently use and would like to look at alot of the details from them and trying to intergrate everything into one form template. These are attached to give an idea as to what we have been using.
