Convert Page to PHP

Hi- We have this test page <[login to view URL]> which is working pretty good now but I would like the page (and the other interior pages) to be in PHP with include files for the header and footer so that if we need to edit the pages someday it is easy for us to do. Also, you'll see the bottom of the file is an image with the copyright info in it. We would like that changed to HTML text. Include in this bid what it would cost to also make changes to these pages (which aren't working quite right, the page above is the correct setup, use that one for the template and apply that to these pages).... <[login to view URL]> <[login to view URL]> <[login to view URL]> <[login to view URL]> <[login to view URL]> <[login to view URL]> Don't worry about the other links on the site, they are part of a third party software. We need to also put into the software a header and footer file similar to the ones attached in the zip file. But it needs to work like the [login to view URL] file above, the files attached contain the old version of the site. So if you could figure that out for us as well that would be great. Let us know what it would cost and when you can do it and hopefully we can work [login to view URL] given to people who can finish it quickly, but we want it done correctly too!
