Auto Database Creation and Manual Database Creation Form

We need a database creation form that has 3 functions. User will be given choice to "Auto Create" database (which will auto create a database and database user), "Custom Create" database (user enters database information manually) and "Remote Database" (which will simply forward user to a url, and skip this page). This application will need a clean HTML5/CSS3 Full Screen design. This is how the application will function: 1. User will come to the page, and will see 3 buttons. "Auto Create Database", "Custom Create Database" and "I Have Remote Database". 2. IF user clicks "Auto Create Database", the form will auto-generate the Database Name, Database Username and Database Password for user and change the MySQL Root Password for the server (the current root password will be the same on all servers, launched by our customers) to a generated password. After this information is generated, user will be forwarded to a page that has the generated hostname (localhost), Database Name, Database Username, Database Password, MySQL Root Username (root) and MySQL Root Password. There will also be a Print button on this page for the user to print the information and a CSS3 rounded button, that allows the user to go the a Install URL (we will leave this URL blank, so that we can change it (per software type). 3. If user clicks "Custom Create Database", a 3 step Jquery form will appear for the user. The first step will be "Changing MySQL Root Password", where the user will enter their new desired root password for MYSQL. The 2nd step, will be the user create the Database Name, Database Username and Database Password. The 3rd Step will be an install button to finish the install. After entering this information, the user will be forwarded to a page with the information they just created (just like in "Auto Create Database") where they can also print the information. This will also have a CSS3 button that allows user to forward to certain software installation. 4. If user clicks "I Have Remote Database", the user will be forwarded to a URL (Which we will change or add) Each time a database is created on the server, whether it is Auto or Custom, this should also be saved to the server in the /home/ directory in a "[login to view URL]" file. For user to get later. After database has been created, whether it is Auto or Custom or if user clicks "Remote Database Install", this form should delete itself. If you can do this job, let us know.
