Access DB for call center with a few data entry fields, reports

Upon reading your project description, I have understand that you are in need of an application that allows to capture user inputs from scripts. I have these following questions regarding the development: 1) What interface would you like this application would run? MS Access or Browser 2) Will this run on MySQL database(server) or just MS Access (file)? if MS access, what version of Office will it run? 3) Is there a standard script to be used? or this will change overtime? 4) Are the users located in just one location? Please be noted that MS Access as Front End would have issues concerning MS Office versions for each user client.
