a web page that retrieve the data on decision tree

need to develop the web page which retrieve the Natural medicine on the database based on the questions answered by the user (e.g if user is allergy , system need to display what kind of allergy do he is having (cold, skin itching, sneezing etc) based on the input he gave if cold then system should display the holy basil plant and brief description, and for sneezing the system must display to the user to take local made honey. this is the brief picture I have given. like this we have to do for entire body.
