A simple PDF file to create - EASY JOB need in 2 hrs

See the file [login to view URL] - just so you can see how you are able to modify the text, and also upload your own pictures to modify the adobe file... That is just an example of the end result I am looking for, an adobe file you can easily modify with the free adobe version with no editing skills required. I would like to create an adobe file like the one I sent earlier that can be edited. I would like something similar to the attached [login to view URL], maybe make it a little fancier looking, and I would like to have it so that: 1. the prices are at the bottom (just like in the [login to view URL]), and can be easily changed with the free adobe reader 2. Where I have all the line items (like create 200 directories, create facebook page, etc) - I would like the line items to be editable - people can type in their own 4. The top - Bronze package, silver package, gold package - I would like those 3 title names editable so people can change them to anything they want 5. I would also like to move everything down some, and then at the top, I would like a field at the top in the middle to insert logo (my logo is in the bottom left on this one, but I would like to move that image to the top middle, and make it so it can be replaced easily) 6. I would like a text field in the top right, for people to enter their company name, phone, website, etc. - make sure this text area is easily editable 7. Same for top left, a place where you can put, proposal prepared for Riverfront Media Group - make this editable as well (a text area in the top left)
