Design a Brochure - A5 Folded Booklet for Application Form - (A4 Double Sided)

Hey there! I have a simple job to design an A5 folded booklet (A4 folded portrait style with front cover - internal with application form inside) and back cover page. No internal pages - just 2 x A4 folder to create booklet style. THe final file must be packaged up - .indd file so i can edit text later. So in total it will be Front and Back cover design and then internal fold out application form. The most time will be spent on doing the internal foldout page with form fields. I have started that in .indd but needs a little more work. Thank you!
